Elevate Youth Lab™

Elevate Youth Lab is our flagship capacity-building model, designed specifically for young people aged 12–20 with disability and those in Out-of-Home Care. Rather than doing tasks for young people, we build the skills, routines, and confidence they need to do things themselves.

Through a structured four-stage progression, from engagement and initiation through to independence achieved, we create the conditions for real, lasting change. Each young person's plan is personalised, evidence-informed, and focused on measurable outcomes that matter to them and their families.

Support Coordination

Our support coordination services help young people and families navigate their NDIS plans, connect with the right providers, and build capacity to manage their own supports effectively.

Community Participation

Meaningful community participation is about connection, belonging, and growth. We help young people engage in social and community activities that matter to them, whether that's sports, creative pursuits, volunteering, or simply being part of their local community.

We build social skills, confidence, and lasting community connections in supportive, inclusive environments. Our approach focuses on natural supports and peer relationships, helping young people develop the independence and confidence to participate in community life on their own terms.

Independent Living Skills

Independence in daily living is built through structured, real-world practice. We support young people to develop essential life skills, from cooking and budgeting to using public transport and managing personal care, in the actual environments where they'll use these skills.

Our approach is evidence-informed, individualised, and focused on what matters to each young person. We don't just teach skills; we build confidence, problem-solving ability, and the resilience to manage life independently.

Skill areas we support:

  1. 1 Cooking, nutrition, and meal planning
  2. 2 Financial management and budgeting
  3. 3 Using public transport and community access
  4. 4 Personal care and hygiene routines
  5. 5 Household management and cleaning

Who Can Access Our Services

Our services are for:

  1. Young people aged 12–20 with disability and NDIS funding
  2. Young people in Out-of-Home Care (OOHC), including those in the care of the Minister
  3. Young people with complex needs including trauma backgrounds
  4. Young people seeking structured, outcomes-focused support
  5. Families and carers wanting professional coordination and capacity building

How to access our services:

  1. Confirm NDIS eligibility or Out-of-Home Care status
  2. Contact us to discuss your needs and goals
  3. Submit a referral form (self-referral or through an NDIS planner or support coordinator)
  4. Participate in initial assessment and planning with our team
  5. Begin structured support tailored to your needs
