Supporting Communities Australia is an NDIS Registered Provider based in Liverpool, NSW, delivering specialist capacity-building and community participation services for young people aged 12–20 with disability and those in Out-of-Home Care across South West and Western Sydney, with a growing vision to reach young people right across Australia.

We are committed to the principles of child safety, trauma-informed practice, and culturally safe service delivery. Every young person we work with has unique strengths, challenges, and aspirations. Our role is to support them in building the independence, confidence, and community connections they need to thrive.

Our flagship program, Elevate Youth Lab™, combines evidence-informed capacity-building with practical community participation. We also offer support coordination, social engagement activities, and independent living skills training, all designed to meet young people where they are and help them move forward at their own pace.

What We Offer

Elevate Youth Lab™

Our structured capacity-building model designed for young people aged 12–20 with disability. Four stages of support help build independence in daily living, social, and community participation skills.

Support Coordination

Helping young people and families navigate NDIS plans, access services, and make the most of funding. We work collaboratively with NDIS planners, DCJ caseworkers, and support networks.

Community Participation

Meaningful social and community engagement activities that build skills, connections, and confidence. From group outings to one-on-one support, we create opportunities for real-world participation.

Independent Living Skills

Practical training in daily living tasks, personal care, money management, cooking, transport, and self-advocacy. Skills are taught in real environments where young people will actually need them.
